RESERVATION RULES

Reservation process at reservation system

  1. Guest has the opportunity to choose the offered packages and accommodation services, which are presented in the booking.gradiali.com reservation table.
  2. After choosing the package/accommodation type and price, Guest can choose the appropriate term for the selected type and additional conditions or the service package he/she feels.
  3. After choosing the service, Guest takes the next step - fills in the form, which indicates his/her personal data and comments (if any) for the reservation he/she likes.
  4. After filling in the data, Guest can make an advanced payment in one of the following ways: by credit card (Visa, Mastercard, Eurocard), electronic bank transfer (UAB Paysera LT), gift voucher (gradialixxxx):
    • When Guest chooses to pay by credit card, Guest is redirected to a page where the payment can be made using the UAB Paysera LT system. Data authorization begins after connecting to the UAB Paysera LT system using an encrypted 128-bit protocol. After UAB Paysera LT system accepts the payment, Guest is automatically notified by e-mail with a letter confirming payment and reservation. Email letter states: Guest details, name of the sanatorium, total cost of accommodation, advanced payment and the amount to be paid at the sanatorium. The remaining amount for the purchased accommodation or services is paid by Guest at the sanatorium. Confirmation letter of the reservation must be submitted to the sanatorium as proof of the advanced payment and the calculation of the remaining amount.
    • If Guest chooses to pay by bank transfer or later, Guest will receive confirmation e-mail of the initial reservation, which will be confirmed after paying in advance by simple bank transfer or using the online system. The reservation is considered initial 48 hours from the moment of its booking and if the advanced payment is not paid within 48 hours, the reservation may be canceled. After receiving the advanced payment, the Guest receives an email confirming the reservation. Confirmation letter of the reservation must be submitted to the Sanatorium as proof of the advanced payment and the calculation of the remaining amount.
    • When Guest chooses to pay with a gift voucher, Guest is directed to the payment section, where after entering the correct data, the payment method "Gift Coupon" is selected next to the payer's information. After entering the correct coupon number and valid PIN code, Guest pays for all or part of the selected services in the shopping cart.
    • Guest, using a personal account in the BookingRobot system, has the option of linking his/her credit card(s) to the account. In this case, every time sanatorium services are purchased, the amount required to be paid will be debited from the Guest's credit account automatically, at the time-of-service purchase. Guest can cancel the linking of the card at any time by clicking the "Delete" button in the "Payment cards" section (system account settings). The security of the Guest's credit card data is guaranteed by UAB Paysera LT, company registered with LB.
  5. Guest understands that in the event of malfunction/programming error in the online reservation system, if, taking into account the composition of the services, the published price of the services is unreasonably low/high, the Sanatorium has the right to inform the Guest of the system error within 10 days from the confirmation of the reservation and demand payment of the full price, and if the Guest refuses to pay the full price - to terminate the contract/refuse to provide services without covering Guest's losses.

Payment and cancellation of reservation:

  1. Guest pays the specified advanced payment using one of the three methods used in the system.
  2. Guest does not pay any additional costs related to the execution of the reservation transaction.
  3. If the reservation is canceled more than 5 calendar days before the arrival date, 75% of the paid reservation fee is returned to the Guest.
  4. If the reservation is canceled less than 5 calendar days before the arrival date, the reservation fee is not refunded.

Rules for ordering a gift voucher:

  1. Gift voucher confirms the gift voucher holder's right to use services for the amount specified in the gift voucher.
  2. Gift coupon is valid only when ordering services directly by phone +370 667 22801 or e-mail. by mail info@gradiali.com.
  3. The holder of the coupon can only use the services of UAB Sveikatos uostas and UAB Palangos Žvorūnė.
  4. If the desired purchase amount of services exceeds the value of the gift voucher, the resulting difference can be paid by credit card or cash.
  5. After the coupon expires, money is not refunded.
  6. The gift coupon (amount) is valid for 6 months. from the date of purchase, unless otherwise stated.
  7. Gift voucher (for accommodation / wellness package) is valid for 3 months. from the date of purchase, unless otherwise stated.
  8. When purchasing a combined gift voucher for an overnight stay service on weekend days, the minimum number of nights is 2 (two).
  9. If the gift voucher has not been used during its validity period, it is considered invalid later.
  10. If the holder of the gift voucher purchases services for an amount lower than that specified in the gift voucher, the remaining amount (difference) is not refunded.
  11. VAT invoice is not issued to the holder of a gift voucher who pays with a gift voucher.
  12. Gift voucher must be used once.
  13. Gift coupon (its value) cannot be exchanged for money.

Personal data

By reserving a package at the Sanatorium, Guest agrees that his/her personal data will be submitted to the database of the serving reservation system. These data are used only to fulfill the reservation procedure and for marketing purposes, in accordance with the applicable rules and consents regarding the protection of personal data.

Final Provisions

  1. Guest, who has familiarized himself with the rules and confirmed them (by ticking "I have read the rules") undertakes that the conditions presented in the rules are acceptable and suitable for the Guest and that he/she agrees with them. Without approval of the rules, the reservation process cannot be completed.
  2. Guest must correctly indicate personal data in the reservation form. The Sanatorium is not responsible for an incorrectly chosen arrival or departure date, for incorrect other data specified by the Guest, necessary for making a correct reservation.
  3. The electronic payment system UAB Paysera LT is responsible for the correct service of reservation money.
  4. The Sanatorium and the online reservation service company are not responsible for the unavailability of the system not due to their fault and for other malfunctions beyond their control.
  5. For other issues not discussed in these rules, related to the purchase of services at https://booking.gradiali.com, the provisions of the internal procedure rules of the sanatorium Gradiali are applied.

Hotel Internal Rules

Bendrosios nuostatos

  1. Sanatorija „Gradiali“ (toliau – sanatorija) – tai poilsio vieta, kurioje teikiamos apgyvendinimo, maitinimo, reabilitacinio gydymo, sveikatinimo ir grožio paslaugos. Taip pat organizuojamos konferencijos, šventės bei kiti renginiai.
  2. Sanatorijos operatorius (toliau – operatorius) – UAB „Sveikatos uostas“ (reg. adresas Vanagupės g. 15, Palanga, įm. k. 303997416) ir UAB „Palangos Žvorūnė“ (reg. adresas Vanagupės g. 15, Palanga, įm. k. 152679144).
  3. Sanatorijos svečias – kiekvienas asmuo, esantis sanatorijos teritorijoje.
  4. Šios vidaus tvarkos taisyklės (toliau – taisyklės) nustato sanatorijos svečių pareigas ir teises naudojantis sanatorijos paslaugomis bei svečių aptarnavimo tvarką.
  5. Šios taisyklės yra privalomos visiems sanatorijos svečiams.
  6. Kiekvienas svečias privalo laikytis šiose taisyklėse nustatytų reikalavimų.
  7. Kiekvienas svečias, registruodamasis sanatorijoje, patvirtina, kad yra susipažinęs su šiomis taisyklėmis pats bei su jomis supažindinęs kartu atvykusius asmenis, ir įsipareigoja jų besąlygiškai laikytis bei užtikrinti, kad jų laikytųsi kartu atvykę asmenys.
  8. Už pasekmes, atsiradusias dėl to, kad šių taisyklių nežinojo ar nesilaikė, atsako sanatorijoje užsiregistravęs asmuo.
  9. Sanatorijos personalas turi teisę neįleisti asmenų, kurių elgesys yra netinkamas, prieštarauja viešajai tvarkai bei kitoms visuomenėje priimtoms elgesio normoms.
  10. Asmenims, kurie nesutinka laikytis sanatorijos taisyklių, sanatorijos paslaugos nėra teikiamos.
  11. Sanatorijos administracija turi teisę pašalinti iš teritorijos svečius, kurie po pirmo perspėjimo pakartotinai pažeidžia sanatorijos taisykles.
  12. Saugumui užtikrinti sanatorijos bendro naudojimo patalpos ir erdvės stebimos vaizdo kameromis.
  13. Sanatorijos svečias privalo atlyginti padarytą žalą sanatorijos turtui bei tretiesiems asmenimis pagal Lietuvos Respublikos teisės aktų nustatytą tvarką.

Apmokėjimas už paslaugas tvarka

  1. Sanatorijos kainas nustato operatorius.
  2. Operatorius turi teisę pakeisti sanatorijos paslaugų sąrašą ir įkainius be išankstinio įspėjimo.
  3. Sanatorija, prieš suteikdama paslaugas, pasilieka teisę prašyti sumokėti avansą (iki 50 proc. užsakomos paslaugos kainos) arba visą užsakomų paslaugų kainą.
  4. Už suteiktas paslaugas galima atsiskaityti iš karto po jų suteikimo arba įtraukti mokėtiną sumą į kambario sąskaitą.
  5. Administraciniai mokesčiai, kainos už papildomas paslaugas ir padarytos žalos įkainiai nustatyti šių taisyklių 1 priede.

Darbo ir ramybės laikas

  1. Sanatorija dirba visą parą.
  2. Nuo 23.00 val. iki 6.00 val. sanatorijoje yra ramybės laikas. Jis gali būti keičiamas tik sanatorijos administracijai leidus. Ramybės laiku negalima triukšmauti ar kitaip trikdyti sanatorijos svečių.

Vaikai

  1. Vaikas – asmuo iki 18 metų.
  2. Vaikai paslaugomis gali naudotis tik kartu su juos prižiūrinčiais suaugusiais asmenimis, kurie atsako už nepilnamečių asmenų saugumą bei šių taisyklių laikymąsi.
  3. Vaikai iki 14 metų į baseinų ir pirčių erdvę įleidžiami tik prižiūrimi lydinčių asmenų, kurie atsako už jų saugumą bei viešosios tvarkos laikymąsi.

Augintiniai

  1. Sanatorijoje augintiniai nepriimami.

Vieta automobilio stovėjimui

  1. Sanatorijos svečiams vieta automobilio stovėjimui yra nemokama.
  2. Sanatorijoje važinėti transporto priemonėmis galima tik tam skirtais keliais. Parkuoti automobilius ant žolės griežtai draudžiama. Už šį pažeidimą taikomas administracinis mokestis – 50 Eur.

Svečių apgyvendinimas

  1. Sanatorijos svečiai atvykimo dieną priimami nuo 15.00 val., išvykimo dieną išregistruojami iki 12.00 val.
  2. Sanatorijos svečiams norint sutrumpinti savo viešnagės laiką, taikomas pusės kitos paros kainos mokestis, nepriklausomai nuo to, kiek dienų trumpinamas viešnagės laikas.
  3. Sanatorijos svečiams norint prailginti viešnagės laiką, t. y., pasirinkti vėlyvą išvykimą, tačiau ne vėliau, kaip iki 19 val., taikomas mokestis – 10 Eur/val.
  4. Sanatorijos svečiams norint atvykti anksčiau, taikomas valandinis mokestis – 10 Eur./val.
  5. Svečias, gyvenantis sanatorijoje, turi sumokėti Palangos miesto mokestį t. y., asmeniui taikomas 1 Eur./d. mokestis.
  6. Registracijos metu svečias turi pateikti asmens tapatybę patvirtinantį dokumentą, užpildyti Svečio registracijos kortelę.
  7. Išvykdamas svečias privalo grąžinti personalui sanatorijos inventorių. Sugadinus, pametus ar kitaip jį praradus, svečias privalo atlyginti inventoriaus vertę. Pametus magnetinį raktą taikomas 11 Eur. administracinis mokestis.
  8. Lankytojas, patyręs turtinę ir/ar neturtinę žalą, nedelsdamas turi pranešti apie tai sanatorijos registratūrai, nurodant įvykio vietą ir aplinkybes. Jeigu lankytojas nesilaiko šios sąlygos, vėliau pateikti nusiskundimai nenagrinėjami.
  9. Kambariuose gali apsistoti tik sanatorijos registratūroje įsiregistravę asmenys. Svečiai turi sumokėti už visus faktiškai kambaryje gyvenančius asmenis. Nepranešus administracijai apie papildomai kambaryje gyvenančius asmenis, taikomas administracinis mokestis – 200 Eur. asmeniui.
  10. Sanatorijos svečias turi teisę nemokamai naudotis kambaryje esančiu seifu.
  11. Sanatorijos patalpose yra griežtai draudžiama rūkyti. Administracinis mokestis už rūkymą balkonuose, kambariuose ir viešose erdvėse – 100 Eur. asmeniui.
  12. Rūkyti galima tik tam specialiai įrengtose rūkymo zonose.
  13. Sanatorijos kambariai yra tvarkomi kasdien nuo 8.00 val. iki 20.00 val.
  14. Sanatorijos patalynė keičiama kas tris dienas, rankšluosčiai ir chalatai keičiami kasdien svečiui pageidaujant.
  15. Rezervavęs kambarį(-ius) asmuo atsako už visų kambaryje gyvenančių asmenų elgesį ir šių taisyklių laikymąsi.

Maitinimo paslaugos

  1. Pusryčiai sanatorijoje teikiami nuo 7.30 val. iki 10.00 val.
  2. Pietūs sanatorijoje teikiamai nuo 12.30 val. iki 14.30 val.
  3. Vakarienė sanatorijoje teikiama nuo 17.00 val. iki 19.00 val.
  4. Operatorius, priklausomai nuo sezoniškumo ar nenumatytų aplinkybių, pasilieka teisę keisti pusryčių, pietų ir vakarienės pateikimo laiką.
  5. Išvykstant iki 7.30 val., svečiui pageidaujant, pateikiamas maisto davinys. Svečias apie ankstyvą išvykimą turi informuoti sanatorijos registratūrą prieš dieną iki 18.00 val.
  6. Tuo atveju, jeigu svečias neturi jokios galimybės gauti jam priklausantį maitinimą (pvz., vėlyvo atvykimo atveju, išskirtiniais atvejais – vykstant į ekskursijas ar pan.), svečiui pageidaujant, jam suteikiamas maisto davinys. Svečias apie šį poreikį turi informuoti sanatorijos registratūrą ne vėliau kaip likus 12 val. iki 44-46 p. nurodyto maitinimo pradžios termino.
  7. Maisto davinį sudaro šalti užkandžiai: sūrio, mėsos, duonos, pieno produktai, vaisiai, vanduo ir gaivusis gėrimas.
  8. Sanatorijos svečiams išsinešti maistą iš savitarnos restorano yra draudžiama (išskyrus cukriniu diabetu sergančius klientus).

Gydomosios ir grožio paslaugos

  1. Sanatorijos svečias procedūros laiką ir pačią procedūrą gali pakeisti nemokamai vieną kartą. Vėliau taikomas 5 Eur. administracinis mokestis už vienos procedūros keitimą.
  2. Sanatorijos svečiui pakarotinas vizitas pas gydytoją ir gydymo plano keitimas yra nemokamas. Tolimesni pakartotiniai vizitai ir gydymo plano keitimai yra mokami. Vieno vizito kaina – 9,00 Eur.
  3. Sanatorijos svečias procedūros rezervaciją atšaukti gali ne vėliau nei likus 3 val. iki procedūros pradžios. Laiku neatvykus į procedūrą ir iš anksto nepranešus pinigai negrąžinami.
  4. Sanatorijos personalas turi teisę nepriimti kliento ir neatlikti procedūros, jei svečias vėluoja daugiau nei 5 min.
  5. Už nepanaudotas neatšauktas paslaugas pinigai negrąžinami.
  6. Apsvaigusiems nuo alkoholio, narkotinių ar psichotropinių medžiagų asmenims gydomosios ir grožio paslaugos neteikiamos.

  1. Baseinų ir pirčių erdvės (toliau – SPA erdvė) darbo laiką nustato operatorius.
  2. SPA erdvės darbo laikas yra aprašytas sanatorijos registratūroje.
  3. Į SPA erdvę svečias gali įeiti su magnetine (kambario) kortele, išduota registratūroje.
  4. Už SPA erdvėje ir drabužinėje paliktus daiktus operatorius neatsako.
  5. Svečiai SPA erdvėje privalo dėvėti neslidžią avalynę.
  6. Svečiai SPA erdvėje privalo laikytis higienos reikalavimų:
  7. visi į SPA erdvę įeinantys ir iš jos išeinantys svečiai privalo nusiprausti duše;
  8. prieš įeinant į pirtis bei jomis pasinaudojus, būtina nusiprausti duše;
  9. SPA erdvės svečiai privalo įsitikinti, kad jų sveikatos būklė ir fizinės galimybės leidžia naudotis teikiamomis paslaugomis. Svečiai patys atsako už savo sveikatos būklę naudojantis SPA erdvės paslaugomis.
  10. SPA erdvės paslaugomis draudžiama naudotis apsvaigusiems nuo alkoholio, narkotinių ar psichotropinių medžiagų asmenims.
  11. Svečiams SPA erdvėje draudžiama:
  12. turėti savo maistą ir gėrimus;
  13. įlipti ir išlipti iš baseinų ne tam skirtose vietose;
  14. šokinėti į vandenį nuo baseinų kraštų.
  15. Lankytojas, padaręs žalą SPA erdvei, atsako Lietuvos Respublikos įstatymų nustatyta tvarka. Už žalą, kurią padaro nepilnamečiai, atsako jų tėvai ar globėjai.

Vaikų žaidimo kambario paslaugos

  1. Sanatorijoje yra įrengtas vaikų žaidimų. Suteikdama šią paslaugą sanatorija neatsako už šios paslaugos įtaką konkretaus vaiko sveikatai, taip pat negarantuoja jos teigiamo poveikio.
  2. Už vaiką atsakingas asmuo, prieš palikdamas vaiką žaidimų kambaryje, privalo įsitikinti, kad vaiko sveikatos būklė jam leidžia būti žaidimų kambaryje bei žaisti jame kartu su kitais vaikais.
  3. Ši paslauga, atsižvelgiant į konkretaus vaiko sveikatos būklę, gali turėti neigiamos įtakos vaiko sveikatai, todėl už vaiką atsakingas asmuo, palikdamas vaiką žaidimų kambaryje, privalo būti atsargus ir rūpestingas, atidžiai įvertinti vaiko psichologinę ir fizinę sveikatos būklę. Neigiamos pasekmės, susijusios su šios pareigos nevykdymu, tenka už vaiką atsakingam asmeniui, palikusiam vaiką žaidimų kambaryje.
  4. Žaidimų kambario taisyklės privalomos visiems asmenims, kurie palieka savo vaikus žaidimų kambaryje.
  5. Vaikų žaidimų kambario administratorius (kai yra) gali atsisakyti priimti vaiką, kuris nuolat jo neklauso ar nedrausmingai elgiasi, gadina žaidimų kambario inventorių, žaloja kitus žaidimų kambaryje esančius vaikus.

Priešgaisrinė sauga

Sanatorijos svečias kilus gaisrui turi:

  1. nedelsdamas pranešti apie tai priešgaisrinei gelbėjimo tarnybai tel. 112 ir sanatorijos registratūrai tel. 90 arba +370 667 22801;
  2. įsidėmėti kambaryje esantį evakuacijos planą, kuriame nurodomi evakuaciniai išėjimai;
  3. pasitraukti iš pavojingos zonos ir veikti pagal sanatorijos personalo arba priešgaisrinės gelbėjimo tarnybos nurodymus.

Baigiamosios nuostatos

  1. Svečių pamesti ir darbuotojų ar kitų svečių surasti daiktai yra perduodami registratūrai.
  2. Sanatorija rastus (paliktus) svečių daiktus įsipareigoja saugoti 30 dienų nuo svečio išvykimo dienos. Higienos priemonės, apatinis trikotažas ir kiti panašūs daiktai nėra saugomi ir išvykus svečiui utilizuojami.
  3. Rašytiniu svečio prašymu sanatorijoje saugomi svečiui priklausantys daiktai gali būti jam išsiųsti paštu ar kitaip perduoti, jei svečias sutinka apmokėti daikto (-ų) siuntimo išlaidas.
  4. Svečias, patyręs turtinę ir/ar neturtinę žalą, turi nedelsiant, ne ilgiau kaip per 1 val., pranešti apie tai sanatorijos registratūros darbuotojui, nurodyti įvykio vietą, laiką ir aplinkybes.
  5. Operatorius neatsako už svečių asmeninius daiktus, paliktus sanatorijos teritorijoje.
  6. Sanatorijos personalas neatsako už be suaugusiųjų priežiūros paliktus vaikus.

Through this Privacy Policy (hereinafter referred to as the “Policy”) we aim to inform you how “Sveikatos uostas”, UAB, company code: 303997416, address: Vanagupės g. 15, Palanga, and “Palangos Žvorūnė”, UAB, company code: 152679144, address: Vanagupės g. 15, Palanga, Lithuania, (hereinafter referred to as the “Joint Data Controllers” or the ““Gradiali” Medical SPA & Wellness”) collects, uses, protects and shares your personal data.

In processing your personal data, “Gradiali” Medical SPA & Wellness shall observe the General Data Protection Regulation (hereinafter referred to as the “GDPR”), the Republic of Lithuania Law on Legal Protection of Personal Data, the Republic of Lithuania Law on Electronic Communications, the recommendations and interpretations of the State Data Protection Inspectorate, as well as other legislation governing the protection of personal data.

This Policy shall apply when personal data is provided during registration for accommodation and other services provided by “Gradiali” Medical SPA & Wellness, as well as when using our Website www.gradiali.lt (hereinafter referred to as the “Website”), when communicating with us by telephone, when visiting the territory of “Gradiali” Medical SPA & Wellness, as well as when concluding service provision or other agreements with us.

Personal data is any type of information about you that can be used to directly or indirectly identify you, such as your name, personal identification number, the date of your visit to the Website and your IP address, as well as other identifiers of your physical, physiological, mental, economic, cultural or social identifiers. 

We process your personal data for the following purposes:

a.      For the purpose of booking accommodation:

i.                    When booking by phone, we collect the following personal data: name, surname, e-mail address, number of guests, number and type of rooms, length of stay, payment details.

Purpose of collecting personal data: booking for accommodation, provision of SPA, wellness, and catering services.

Legal basis for processing of personal data: it is necessary to provide this personal data for the purpose of making and fulfilling the order/registration for accommodation/service provision (in order to enter into a service provision agreement) (Article 6(1)(c) of the GDPR).

Personal data provided during booking/registration by phone is stored for 2 months from the date of provision.

ii.                   When booking by e-mail or via the www.gradiali.lt (booking.gradiali.com) Website, we collect the following personal data: name, surname, e-mail address, number of guests, number and type of rooms, length of stay, payment details.

Purpose of collecting personal data: booking for accommodation, provision of SPA, wellness, and catering services.

Legal basis for processing of personal data: it is necessary to provide this personal data for the purpose of making and fulfilling the order/registration for accommodation/service provision (in order to enter into a service provision agreement) (Article 6(1)(c) of the GDPR), as well as the fulfilment of a legal obligation (statutory obligation under the Republic of Lithuania Law on Tourism) (Article 6(1)(b) of the GDPR).

Personal data is stored for 10 years from the date of booking/registration.

iii.                 When checking in at the reception desk, we collect the following personal data by providing you with a registration form for completion: name, surname, passport No., date of birth, accompanying guests (name, surname), nationality, phone number, vehicle license plate No., room No., period of stay, signature; details if an invoice is required.

Legal basis for processing of personal data: it is necessary to provide this personal data for the purpose of making and fulfilling the order/registration for accommodation/service provision (in order to enter/when entering into a service provision agreement) (Article 6(1)(c) of the GDPR), as well as the fulfilment of a legal obligation (statutory obligation under the Republic of Lithuania Law on Tourism) (Article 6(1)(b) of the GDPR).

Personal data provided at the reception desk at the time of check-in/registration is stored for 5 years from the date of completing the registration form.

 

b.      For the purpose of registering for the provision of wellness and SPA services/procedures:

i.                    When registering by phone or e-mail, we collect the following personal data: name, surname, phone number, year of birth, gender, wellness services/procedures chosen.

Purpose of collecting personal data: registration for the provision of wellness and SPA services/procedures.

Legal basis for processing of personal data: it is necessary to provide this personal data for the purpose of making and fulfilling the order/registration for the provision of services (in order to enter into a service provision agreement) (Article 6(1)(c) of GDPR).

Personal data provided during registration by phone is stored for 2 months from the date of provision.

Personal data provided by e-mail is stored for 2 years from the date of provision.

ii.                   When checking in at the SPA or treatment reception desk, we collect the following personal data by providing you with a registration form to complete: name, surname, telephone number, year of birth, gender.

Purpose of collecting personal data: registration for the provision of wellness and SPA services/procedures.

Legal basis for processing of personal data: it is necessary to provide this personal data for the purpose of making and fulfilling the order/registration for the provision of services (in order to enter into a service provision agreement) (Article 6(1)(c) of GDPR).

Personal data provided at the reception desk is stored for 2 years from the date of provision. 

c.      “Gradiali” Medical SPA & Wellness processes your health data when you are provided with inpatient or outpatient rehabilitation, as well as therapeutic services (subject to a doctor's consultation). During the provision of therapeutic services, the following personal data is collected: name, surname, personal identification number, address, diagnosis, social status, referring institution and physician, assigned physician, profile, examination description, procedures prescribed, treatment applied, procedures purchased.

Purpose of collecting personal data: provision of therapeutic services.

Legal basis for processing personal data: your consent to the processing of your health data (Article 9(2)(1) of the GDPR).

Personal data processed for the purpose of outpatient rehabilitation is stored for 15 years from the date of provision. 

Personal data processed for the purpose of inpatient rehabilitation is stored for 25 years from the date of provision. 

 

d.      “Gradiali” Medical SPA & Wellness processes your name, surname, payment details, e-mail address and phone number for the purpose of providing you with a gift voucher purchase service.

Legal basis for processing of personal data: it is necessary to provide this personal data for the purpose of making and fulfilling the purchase order (in order to enter into a service provision agreement) (Article 6(1)(c) of the GDPR).

Personal data is stored for a period of 10 years from the date of registration.

 

e.      Direct marketing carried out by “Gradiali” Medical SPA & Wellness is:

-          news, commercial offers and surveys communicated by e-mail;

-          news, commercial offers and surveys communicated by phone.

The legal basis for the processing of personal data: consent given by the person for the processing of personal data for direct marketing purposes or the legitimate interest of “Gradiali” Medical SPA & Wellness, under the conditions set out in the Republic of Lithuania Law on Electronic Communications.

For the purpose of direct marketing, the following personal data provided by the person is processed: name, surname, e-mail address, phone number, company name.

Personal data is processed for the purpose of direct marketing is stored for a period of 3 (three) years from the receipt of consent.

The person giving consent shall always have the right to refuse all or part of the direct marketing offers at any time. This can be done by clicking on the link in the received newsletter/offer that allows you to opt-out of receiving all or part of the direct marketing offers. You can also opt-out of direct marketing offers by sending an e-mail to marketing@gradiali.com. Opting out of direct marketing offers will not have negative consequences, but you will no longer receive relevant offers, news or other direct marketing information in the future.

Revocation of consent shall not affect the processing of personal data already carried out prior to the revocation of consent or prior to the statement of refusal to consent to direct marketing and the consequences of such processing.

 

f.        “Gradiali” Medical SPA & Wellness processes call recordings for quality improvement and complaint handling purposes.

Phone calls are recorded when:

-          the person calls “Gradiali” Medical SPA & Wellness him/herself;

-          the employees of “Gradiali” Medical SPA & Wellness call the person.

The legal basis for the processing of personal data is consent to the recording of the call. Consent to the recording of calls is given when the person continues the conversation (when the person calls “Gradiali” Medical SPA & Wellness) or confirms that he or she consents to the recording of the conversation (when “Gradiali” Medical SPA & Wellness calls the person).

If the person does not consent to the recording of the call, he/she may contact “Gradiali” Medical SPA & Wellness in other ways: by e-mail info@gradiali.com or by visiting “Gradiali” Medical SPA & Wellness.

For the purpose of recording calls, the following personal data is processed: phone number, name, date of the call, start and end time of the call, content of the call.

Call recordings are stored for 2 months after the recording.

 

g.       “Gradiali” Medical SPA & Wellness carries out video surveillance on the territory and premises of “Gradiali” Medical SPA & Wellness.

Video surveillance is carried out to ensure the protection of property rights, the security and integrity of property and the safety of employees and guests.

The processing of video recordings is based on Article 6(1)(f) of the Regulation, i.e. the processing is necessary for the purposes of the legitimate interests of the Data Controller or a third party.

Video recordings are stored for 30 calendar days.

 

h.      “Gradiali” Medical SPA & Wellness processes the personal data of clients, suppliers, vendors and/or their representatives for the purposes of commercial relations, conclusion and execution of agreements.

For this purpose, personal data is processed on the basis of the legitimate interest of the “Gradiali” Medical SPA & Wellness and its clients, vendors and suppliers to conclude and properly execute agreements.

Personal data processed for this purpose include: name, surname, workplace, position, phone number, email address.

The provision of this personal data is necessary for the conclusion and execution of the agreement.

Personal data is stored for 10 years from the expiry of the agreement. This period shall be extended for a further 5 years if the commercial relations with the above-mentioned clients, suppliers and vendors are continued.

To whom your personal data is provided:

-          law enforcement authorities, courts and state institutions in accordance with the procedure provided for by the legislation of the Republic of Lithuania;

-          other third parties (data processors) who process and/or have access to your personal data on behalf of and under the instructions of the Company, e.g. service providers involved in the development, maintenance and support of booking systems and/or other persons who assist “Gradiali” Medical SPA & Wellness in the proper provision of services to you;

-          entities providing legal and insurance services, where disclosure of such personal data is necessary to establish, exercise or defend the rights and legitimate interests of “Gradiali” Medical SPA & Wellness;

-          to the partners of “Gradiali” Medical SPA & Wellness in cooperation with whom you are provided services, as well as to any third parties, in accordance with the cases and to the extent provided for by legislation.

“Gradiali” Medical SPA & Wellness shall take appropriate measures to ensure that the processors it engages process the personal data entrusted to them only for the purposes specified by “Gradiali” Medical SPA & Wellness, carry out only those actions that “Gradiali” Medical SPA & Wellness has instructed them to carry out, and ensure appropriate organisational and technical measures for the security of personal data.
